Eligibility: Hospice patient and family members. Families who come to Iowa City Hospice for help, receive services customized to their needs.Service Description: Iowa City Hospice provides a full range of supportive services for terminally ill patients and their loved ones. The interdisciplinary teams focus on physical, spiritual, and emotional comfort. Able to provide pain and symptom management for patients and enhance communication and confidence for caregivers. AKA: N/ABackground Checks: YesComments:*Hospice care may be provided wherever the patient calls home, which could be a relative’s house, a hospital, a long-term care facility (nursing home), or even a hotel.*Services include medical care, pain and symptom management, home nurse visitation, care management, counseling and bereavement services for the patient and members of the family.*Grief Support Group and grief education classes available.
