Eligibility: Individuals that have elected to stop curative treatment or those who are expected to be within the last 6 months of life. Must have doctor referral.Service Description: Focused on creating a life full of comfort, dignity, and peace for people in the final stages of life.AKA: St. Luke's HospiceBackground Checks: Yes; Employees.Comments:*Services include: arranging for medical equipment, supplies and medications, managing pain and symptoms, and providing spiritual care, respite care, and bereavement services.*A hospice care team typically includes a doctor, nursing staff, aids, counselors, social workers, pharmacists, volunteers, therapists for Speech, Physical, and Occupational Therapies.*Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in a long-term care facilities such as nursing homes.
