Eligibility: Open to all who have terminal diagnosis and 6 months or less to live. Must be doctor certified.Service Description: Programs that provide a full range of supportive services for terminally ill individuals who are in the final states of their illnesses and for the their families. AKA: N/ABackground Checks: YesComments:*Hospice team typically includes: physician, medical director, nurses, home health aids, therapists for music, massage, and physical needs, as well as volunteers and clergy or counselors.*Focused on pain and symptom management.*Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in a long-term care facilities such as nursing homes.*Office hours listed; services provided 24/7.
