Eligibility: Open to individuals of all ages with disabilities and mental health needs in Eastern Iowa.Service Description: Provides day programs and employment services in Cedar Rapids, Iowa City, and Monticello areas. The mission is to partner with people who have disabilities and mental health needs to advance their personal independence, dignity, and growth.AKA: SUIBackground Checks: YesComments:*Employment Systems: Provides individualized services and supports for adults with developmental disabilities and similar service needs. Serves as a day habilitation program that promotes relationships, self-expression through art, volunteerism, and visiting area attractions. Also helps seek jobs in the community and provided any support needed to maintain jobs.*Supported Living: Provides in-home support by working to set and achieve lifelong goals and learn day-to-day tasks such as cleaning and grocery shopping.*Behavioral Health Solutions: Provides individualized quality behavioral, mental health, and clinical health services. The goal is to assist individuals and their families in meeting the challenges they face to become more successful in all settings. *Approved to provide services through Medicaid Waivers.
