Eligibility: Residents in Greene County- elderly, disabled, ill or convalescent individuals in their homes. Service Description: Provide a broad range of diagnostic medical care and treatment services in persons home or at public health office.AKA:Background checks: Yes - employeesComments:*Hold blood pressure checks every Monday from 12-1:30 PM*Hold flu shot clinics-seasonal.*Need to call to get information and get appointment set up.*Offer Matter of Balance. Falls prevention program*provide care coordination to prevent gaps between healthcare providers.*Greene Co Medical Center Public Health no longer provides home health services (therapy, home care aide, skilled nursing) that are funded by Medicare, Medicaid, VA, or private insurance.* Instead, we offer “safety net” services for Greene County residents who need assistance NOT covered by those funders. Our services are provided on a sliding fee scale according to each client’s ability to pay. Grants from Elderbridge, IDPH, and county tax dollars help to offset the cost for these services. We also provide services for those clients who are eligible for the Elderly Waiver under Medicaid. Available services include:• Skilled nursing visits for acute needs• Skilled nursing visits for chronic needs (Health Maintenance)• Homemaker• Home care aide (personal cares assistance)• Lifeline: Includes basic service, Auto-alert (falls detection) and GPS• Loan closet: Home care equipment such as canes, walkers, commodes & bath benches• Care coordination to prevent gaps between healthcare providers• Matter of Balance: Falls prevention program*email use online form from website
