Eligibility: Veterans and their families.Service Description: Provides health care services in addition to providing assistance to Veterans and their families with connecting them to potential benefits they may be eligible for.AKA: N/ABackground Checks: YesComments:*Provides primary care, mental health care, specialty care services, as well as social programs and services.*Homeless Veteran care available for Veterans who are homeless or at risk of becoming homeless.*Caregiver support program available for those caring for a Veteran. For more information on trainings, counseling, or respite care qualifications please call: 319-688-6933.*The Disabled Veterans (DAV) program provides free van rides to and from the Iowa City VA Medical Center for Veterans who do not have other transportation options. Call: 319-339-7061 for the Cedar Rapids Metro Area. For Benton County Veterans call Benton County VA Office: 319-472-3150. For Monticello/Anamosa area call Dubuque County: 319-499-7570 or 563-589-7840. Must be able to get in and out of the vehicle independently.*Veterans Transportation Services (VTS) provides transportation for Veterans and their caregiver to and from the Iowa City VA Hospitals.
