Eligibility: Open to anyone in need of service.Service Description: Provides holistic and performance based therapy to help active adults return to their previous activities that they enjoyed as quickly and safely as possible.AKA: N/ABackground Checks: N/AComments:*Phyisical and Occupational Therapies.*Home modifications, pain management, group fitness classes, return to home assessments and ADL functions also available.*Accepts Medicare Part B and most other insurances.*Has contracts with various independent and assisted living facilities. Will do home visits as well.
