Description
Eligibility: Patients who need skilled services, are currently under the care of a physician, and have met with a physician within the last 90 days may qualify for services while recovering at home from a recent injury, surgery, hospitalization, or illness. Additionally, Medicare and some insurances require patients to be homebound, which means it is difficult for the patient to leave home, assistance to do so is required, and/or a doctor has advised and placed limitations on activities. If there are questions about the referral process, patient eligibility, or insurances that are accepted, please feel free to contact us. Service Description: Referrals must be made by phone, fax, or use of EPIC by a medical provider. Once a referral is accepted, our team will assess patients in their home and work closely with their health care provider to develop an individualized care plan that meets the patient's health care needs and recovery goals. We are able to provide skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work services, and home health aide services as needed.
