Eligibility: Individuals that have elected to stop curative treatment or those who are expected to be within the last 6 months of life. Must be doctor certified.Service Description: Aims to reduce suffering and promote comfort at the end of an individual's life.AKA: N/ABackground Checks: N/AComments:*Spiritual care and support as well as physical, dietary, and other therapy services.*Managing pain and other symptoms.*Respite care, as well as on-going support for family caregivers.*Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in a long-term care facilities such as nursing homes.*Office hours listed; services available outside of these hours.
