Description

Eligibility: Open to anyone experiencing concerns of memory loss due to dementia or normal aging.Service Description: Health Care Clinic specializing in memory. Completes an evaluation and works with a team of providers to develop care plan.AKA: N/ABackground Checks: N/AComments:*A physician's referral to the clinic is not required.*Patient will complete an evaluation and clinic team will assess the patient's abilities by interacting wit them and their family member and/or friend.*Clinic team meets with patient, family members, and other referral sources after evaluation. Patient will receive a binder of information about their diagnoses and other helpful information.
