Eligibility: Must be 55 yrs or older and must qualify within assisted living guidelines (required needs and available services must be a match).Service Description: Assisted living private apartments that are designed for seniors that need individually assessed supportive services and plans in order to live independently. Chaplain on staff. AKA: noneBackground Check: Performed on all employees.MCOs: Amerigroup, Iowa Total Care, UHCComments:* Amenities: Bistro, Secure Entry System, Kitchens: Apartment size microwave and refrigerator, large walk-in closets, smoke alarms/detectors, individually controlled heat and air.* LEVELS OF CARE Level 1: 24 hour on site staff; two assisted showers per week; monitoring health conditions; tenant responsible for supplying, storing and managing meds. Level 2: All service of Level 1 plus: therapeutic bath once a week; management of health conditions due to illness; 1/2 hour each day of personal assistance; meal time and activity reminders; stand-by assist for transfers.
