Description

Eligibility: Individuals with a mental illness or developmental disability. Must meet all of the county requirements since they are county funded.Service Description: Residential homes or facilities that offer personal care and individual attention for older adults, people with disabilities and other populations whose limitations prevent them from living alone. AKA: CVRBackground Checks: YesComments:*Adult RCF for mentally ill on DD. We assist and supervise the clients with ADLs, shopping, activities in house, and in the community. *Transportation to any appointments.*Serve as payee for majority of clients and assist with budgeting and bill paying.*Medical doctor and Psychiatrist come to facility once a month and are always available on call. *Laundry services and three meals per day provided.*Assist with transition into community living.*Fitness equipment on site, but also transport to other businesses for workouts.*Dog on site, but they cannot bring any of their own in.*Medication administered.*2 single bed wards, 4 double bed wards, and the rest are multiple bed wards.*Will accept patients with Medicaid. When this funding is gone, they will use county funding to help cover the rest of the costs.
