Eligibility: Individuals 18+ with a disability. Service Description: Maintains information about community resources for the disabled and their caregivers which links individuals who are in need of specialized services with appropriate resources and/or which provide information about community agencies and organizations that offer specialized services.AKA: N/ABackground Checks: Yes; Employees.Comments:*Day Habilitation: Available for adults over 50 with age-related deficits or decline, including Alzheimer's and other memory impairments who need supervision and care during the day.*Supported Community Living Services: Assists individuals with completing household tasks (cleaning, laundry, cooking, etc), money management, filling out paperwork, going through mail, etc. Do not provide 24 hour services.*Accepts private pay, Medicaid HCBS Waivers, and Region Funding.*Office hours listed; Adult Day available on Tuesdays, Thursdays, and Fridays from 9:30 AM - 3:00 PM.
