Eligibility: Must be on Medicaid or be getting qualified for Medicaid. Must have a serious mental illness. SMI defined as Psychotic Disorder, Schizophrenia, Schizoaffective Disorder, Major Depression, Bipolar Disorder, Delusional Disorder, and Obsessive-Compulsive Disorder.Service Description: Coordinated, patient-centered Mental Health Care for Adults with serious mental illness. Health services are integrated and coordinated among service providers to provide care coordination that will provide better overall health outcomes.
