Eligibility: Open to individuals of any age with a disability.Service Description: Offers a variety of programs and services that help enhance the lives of people with intellectual and developmental disabilities.AKA: RFYPBackground Checks: YesComments:*Supported Community Living: Hourly or daily services provided in the home and community to adults with disabilities for independent living. Services are provided in the person's home and community, and are available 24 hours per day, 7 days per week. Assistance is provided to complete personal goals and supports such as supervision, personal cares, medical cares, transportation, financial, socialization, cooking, cleaning, grocery shopping, running errands, and activities of daily living. Funding sources accepted include: Intellectual Disability and Brain Injury Waivers, Region, and private pay.*Home Based Habilitation: Services provided in the home and community to assist adults with disabilities with acquiring, retaining and improving the self-help, socialization, and adaptive skills necessary to reside successfully in residential and community-based settings. Services are provided in the person's home and community, and are available 24 hours per day, 7 days per week. Assistance is provided to complete personal goals and supports such as supervision, personal cares, medical cares, transportation, financial, socialization, cooking, cleaning, grocery shopping, running errands, and activities of daily living. Funding sources accepted include: Habilitation and Region.*Day Habilitation: Operates Monday through Friday, 8:00AM-4:00PM. Services provided combine medical monitoring, daily living skills development, and active community involvement to support the development of interpersonal relationships, pre-employment skills, and/or educational development for the person served. Transportation may be provided or arranged through paratransit. Funding sources accepted include: Intellectual Disability and Brain Injury Waivers, Habilitation, Region and private pay.
