Description

Eligibility: Physician referral required for nurse to do evaluation. Patients with life expectancy of 6 months or less and when they have opted to end curative treatment.Service Description: Hospice care consists of intermittent nursing visits to assess and treat symptoms and be sure patient's/families goals are met. If symptoms/pain are difficult to manage, continuous care may be provided.AKA: Background checks: Yes - employeesComments: *Management of pain and symptoms; assistance with emotional, social and spiritual aspects of the end-of-life experience. Provides necessary medications, medical supplies and equipment; educates family and other caregivers; recommends special services when needed; enhances physical care with art, music, massage or pet therapy; coordinates short-term in-patient care or respite if needed; provides bereavement care and counseling to surviving family members and friends. *Staff includes social worker, chaplain, aides, nurses, volunteers, people who do massage therapy and music therapy.*On call nurses available 7 days a week/24 hrs a day.*Jamie is HIM Scheduler in Panora*Office hours listed; services 24/7*Panora office serves 8 county area.
