Eligibility: Physician referral required for nurse to do evaluation. Patients with life expectancy of 6 months or less and when they have opted to end curative treatment.Service Description: Hospice care consists of intermittent nursing visits to assess and treat symptoms and be sure patient's/families goals are met. If symptoms/pain are difficult to manage, continuous care may be provided.AKA: Background checks: Yes - employeesComments: *Management of pain and symptoms; assistance with emotional, social and spiritual aspects of the end-of-life experience.
