Eligibility: Adults with chronic mental illness.Service Description: Provides Home and Community Based Services (HCBS) and Habilitation (HAB) Housing to clients in Johnson and Linn County.AKA: N/ABackground Checks: Yes; Employees.Comments:*8 HAB homes available in Linn and Johnson Counties.*Additionally, provides outreach services to clients living in their own homes.*Services are designed to help individuals work on daily living skills, mental and physical health goals, socialization, and other adaptive skills necessary to live independently in the community.*Day Habilitation coming soon.
