Eligibility: Elderly or DisabledService Description: A skilled nursing facility AKA: NoneBackground checks: YesComments:*No longer do Respite, Out of Home care to help care for a few hours or overnight. *May do short term for week or longer to help out. Call to check (Linda/offc mgr however still listed on website) 5/7/15/kh*8am-4pm when office answers phone. Nurses or others answer other hours, A 24 hr care facility.*Contract in physical, occupational and speech therapy from outside. *Have own inside PTA *Do have van for transporting residents for activities but not used to get to appointments.
