Eligibility: Age 18 and older needing assistance Service Description: Individuals needing some assistance no longer able to live on own, have 3 levels of care based on individual needs.AKA:Background Check: Yes on residents as well as employeesComments:*14 assisted living units with various levels of services. *Try keep residents as independent as possible.*Help with HUD paperwork*There is a storm shelter at this location.*Phone/cable included in the rent*24 hr staffing response. Residents wear call pendants answered by staff on duty all 7 days/wk 6:30 AM-8:30 PM after that calls go to care center staff next door at Rehabilitation Center of Hampton connected to Leahy Grove who check on residents if need assistance.*Haley in office Monday thru Friday 8 AM - 4PM*Social worker shared with nursing home*Alzheimer Support Group meets at Leahy Grove on 3rd Wednesday of month at 4 PM *Secured Alzheimer unit is at the care center (Rehab Center of Hampton) *Grocery Delivery: staff go each Friday to store for residents. Take grocery list, money, shop then return with items and their change.*Pharmacy Delivery: Pharmacies will deliver meds to independent individuals. Those that have med monitoring Leahy Grove handles the meds.
