Eligibility: Elderly or disabled still independent but want some assistance with daily living skillsService Description: Housing with assistance with daily livingAKA: n/aBackground Check: Yes on employeesMCOs: Amerigroup, Iowa Total Care, UHCComments:*Assistance with meals, light housekeeping, laundry, etc.*Can provide short-term basis to individual needing care of recuperation from hospitalization doctor ordered. Usually have 1 or 2 rooms available for this. *Out-patient Therapy clinic, medical clinic, pharmacy located on lower level. *Staff on duty 24/hrs a day.*Can accept payment sources from HUD, Section 8 rental housing thru North Iowa Regional Authority in Mason City if qualifiy.*Service Coordinator -Cheryl Sand, RN*Have activity director/ dietary manager*Transportation: prefer family provide but can arrange with taxi or transit system do need to schedule ahead.
