Eligibility: Have a mental health diagnosis; live in either MahaskaCounty; meet/comply with criteria for financial eligibility requirements (income and resources); will need to provide proof of insurance/application, proof of all household income, photo ID, Social Security Card, proof of disabilityService Description: Determines eligibility for funding, provide specific information regarding services and supports for people with disabilities in the local area, and will assist with referrals to service providers. AKA: n/aBackground Checks: YesComments:*Applicants complete an application with CPC Administrator. If applicant does not have health insurance, they must also fill out the application for the Affordable Care Act.*Case Coordinator sets up funding and coordinates services for the client.*Closed 12:00-1:00 PM
