Description

Eligibility: People who have a need for mental health services; chronic mental illness; Intellectual Disability; Developmental Disability, and others. Based on household income and number in household. Applicants complete an application with CPC Administrator. If applicant does not have health insurance, they must also fill out the application for the Affordable Care Act.Service Description: Determines eligibility for funding, provide specific information regarding services and supports for people with disabilities in the local area, and will assist with referrals to service providers. AKA: Background Checks: Performed on all employees.Comments:* Case Coordinator sets up funding and coordinates services for the client.
