Eligibility: Intended for individuals who have no insurance or covered with Medicaid. Individuals must have a mental health, intellectual disability or developmentally disabled diagnosis Service Description: Assist in determining eligibility for funding, provide specific information regarding services and supports for people with disabilities in the local area, and will assist with referrals to service providers. AKA: Background Checks: N/AComments:*Case Coordinator sets up funding and coordinates services for the client.*Will only be at the office by appointment
