Description

Eligibility: Must be 55+, meet nursing facility level of care, live in specified counties, and be able to live safely in community at the time of enrollment.Service Description: Helps participants live as independently and healthy as possible in their own home.AKA: PACEBackground Checks: N/AComments:*Provides primary care and specialty medical care.*Services may include: medications, therapeutic recreation, dietary services, in home support and care, social work services, respite care, and more.*Transportation to and from PACE center.*Participants may or may not be required to pay a premium for services, depending on their Medicare/Medicaid eligibility.
