Eligibility: Doctor ordered, meet before be sure can provide services neededService Description: 24 hr skilled nursing facility, rehab, respite care, inpatient and out patient therapy contracted in.AKA: n/aBackground checks: Yes on residents and employeesComments:*A skilled nursing facility with private rooms, rehabilitation care, accommodate IV's and Bariatric. *No longer have a secured Alzheimer's unit but may be able to provide care they check dementia case by case to see if can provide this care. *Contract with two hospice agencies for hospice care,respite out of home care to help out caregivers, have occupational & physical therapy inpatient/ outpatient but outpatient need doctor referral. *July 2008 adding new home health care on will go to people homes to do homemaker/personal care services. *Nov-Dec 2008 adding residential -followup on these later on. * 6/15/09 Add Respite in home and Respite out of home(home care staff go out to home to care for folks)they call it Home Health Care & Home Aides thru Living Well at Good Samaritan in Preston, MN.
