Eligibility: Anyone who has an illness or is disabled. Service Description: Provide in home services of RN's and LPN's for persons who need medical care such as recently discharge patient or elderly, frail or disabled person. Also for Hospice patients.AKA: Trinity Regional Home CareBackground checks: YesComments:*Patient nursing care that is ordered by a doctor go out a 50 mile radius of Ft Dodge.*Registered nurse on call 24hrs a day. *Service for individual or family members with physician approval*Additional services as needed include: social workers, occupational therapist, speech and/or physical therapists. *Home-aid (personal care with 45minute time) if time permits can throw in laundry or whatever needed*Have therapy & social worker services.*Home Care Services and Hospice office together at same location to arrange services.
