Eligibility: Open to individuals with disabilities. Accepts ID and BI Medicaid Waivers and Private Pay.Service Description: Serves individuals with disabilities by building independence through community integration, education, support, and advocacy.AKA: N/ABackground Checks: N/AComments:*Provides outreach supported community living (SCL) to individuals with intellectual and developmental disabilities, brain injuries, and chronic mental illness, who are living in the community. Staff works 1:1 with individuals to reach their goals.*Also provides 24-hour daily supports in residential homes where 3-4 individuals reside.*Support includes: medical, community integration, daily living skills, personal care, social skills, and safety.*Transportation to church, grocery store, work, medical appointments, or other errands/social events. *Office hours listed; services available outside of these hours.
