Description

Eligibility: Open to any terminally ill patient and their family within the 7 county service area.Service Description: Programs that provide a full range of supportive services for terminally ill individuals who are in the final states of their illnesses and for the their families. Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in a long-term care facilities such as nursing homes. AKA: N/ABackground Checks: YesComments:*Office hours listed, on-call nurse available 24/7.*Bereavement support groups, counseling, hospice care, volunteer services, and chaplain.*All services related to the primary patient diagnosis are provided free of charge. *Services include but not limited to nursing, CNA, Spiritual care, Bereavement care, Medical Social Work, Volunteers, Massage, Music Therapy, medical director oversight, pharmacy, and durable medical supplies.
